24 Hour Nurse Line
In the primary care setting, offices are not open 24 hours a day, 7 days a week. To combat this downtime, and potential inappropriate use of emergency rooms, RosenCare contracted with a 24-hour registered nurse call center. It provides over-the-phone assessments, guidance, and direction for our patient population. Our medical team receives and reviews a report the morning following the over-the-phone assessment and follows up with the patient as needed.
In-House Medications (30 & 90 day supplies)
The Rosen Medical Center provides an on-site pharmacy to dispense in-house medications which optimizes compliance, increases the probability of patients taking medications, reduces the patient’s travel to other pharmacies, eliminates administrative costs from its pharmacy benefit manager, eliminates dispensing costs from the retail pharmacy distributor, and enhances the service provided to its patients.
Nurse Case Management & In-Patient Hospital Care
RosenCare recognizes that in-patient hospitalization is the largest contributor to overall spending in a health budget. Our in-patient management program reduces your average length of stay and your days per 1,000 covered lives. The case manager is responsible for monitoring patient progression, communicating with the patient about service and needs, coordinating discharge, and coordinating transition to step-down care. Step-down care includes transitioning the patient to home care, skilled nursing facilities, long-term care facilities, or hospice as deemed appropriate.
Pregnancy Care Management
A healthy pregnancy is a goal for all under the RosenCare model. The RosenCare model provides comprehensive care for the prevention of premature complications, low birthweights, infant mortality, and complications for the mother. A nurse case manager is assigned to work with the expectant mother from the start of her pregnancy through the delivery of the baby. Our expectant mothers receive free prenatal vitamins, free breast pumps, educational programs, and access to alternative sites for delivery such as a birthing center and home deliveries.
Triage Program
RosenCare ensures each patient receives access to a facility or team appropriate for his or her needs. Our triage team assesses the patient either on the phone or on a walk-in basis to determine if the facility can provide the immediate appropriate care or if another service, such as an urgent care facility, emergency room, or 911, is needed.
